Employees are expected work in a manner that demonstrates a commitment to quality, patient safety, employee engagement, innovation, and the highest standards of personal integrity, professionalism and competence.OUR CORE VALUESExcellenceIntegritySustainabilityRespectCompassionQualificationsESSENTIAL SKILLS AND EXPERIENCE:Bachelor's Degree in Social Service field is required or a minimum of five (5) years of behavioral health experience and one (1) year of direct experience may replace Bachelor's DegreeHigh school diploma or GED from an accredited institution is required Minimum of one (1) year of experience working with State AHCCCS and Regional Behavioral Health Authority (RBHA) clients; preferredExcellent customer service skillsExcellent computer skillsKnowledge of Arizona Department of Health Services and RBHA requirementsThorough knowledge of current versions of DSM and ICDAbility to complete a Behavioral Health Assessment with appropriate clinical oversightAbility to provide effective and respectful quality care and services that are responsive to diverse cultural beliefs and practicesCPR and First Aid certifiedBilingual English/Spanish strongly preferred Valid Arizona driver's licenseFingerprint clearance cardPOSITION REPONSIBILITIES/ESSENTIAL Participates as directed in the screening of new service requests within the center ensuring that all individuals are adequately and appropriately served according to their individual needs and in accordance with existing policies and proceduresParticipates in regular interdisciplinary staff meetingsAccepts newly assigned cases in which the diagnostic intake has been completed by the Intake Specialist. Completes case management assessment on all clients entering caseloadDevelops appropriate treatment/service plans with clients as the means for implementing appropriate service and developing effective alliances with clientsProvides direct client services to individuals and/or family members. Services provided may include case management, skills training, group counseling, group skills training, health promotion, crisis intervention, transportation, supportive employment services, counseling with appropriate clinical oversight and others depending on client need. Services may be provided in office, home, school or community settingsProvides on-going supportive and/or case management functions in accordance with the problems, needs and the strategies identified within the service plan in order to assist the client to achieve goals and objectivesProvides face to face reviews with the client and/or relevant behavioral health staff on a regular basis. Ensures progress made is reported so that the service plan can be modified as necessary to ensure that the goals and objectives are being achieved. The frequency reviews will be determined by relevant requirementsAssist supervisory staff with tracking of clients, including the generation of reports that will allow follow-through on quality of careDocuments all service contacts on a timely basis including face-to-face interviews, collateral and networking contacts, correspondence and maintains the case records in accordance with internal and regulatory standards requirementsMaintains close communication with the consulting psychiatrist and the Behavioral Health team for input regarding medication compliance, side effects of medication, and medication changes and alerts behavioral health team of any changes in client adjustment which might suggest decompensation and a need for more aggressive interventionPerforms other duties as assigned by the supervisor which are consistent with the position and in compliance with Adelante Health Care policies and proceduresNONESSENTIAL SKILLS AND EXPERIENCE:Experience with community resources and agencies; preferredAdditional Duties and ResponsibilitiesOther duties as requested or assigned
