Job Description Physician – Integrated Primary Care (Onsite)
Relocation assistance to Peoria, AZ available if needed We’re
looking for a dedicated, patient-centered physician to join a
growing clinical team committed to delivering exceptional care in a
supportive, team-based environment. This role offers the
opportunity to practice high-quality medicine with the time,
technology, and collaboration you need to make a meaningful impact
on patient outcomes. What You’ll Do: As a key clinical team member,
you’ll provide direct patient care while contributing to innovative
quality and population-health initiatives. You’ll help shape a
patient experience grounded in trust, accessibility, and clinical
excellence. Responsibilities include: - Evaluating and treating
patients in alignment with established clinical standards. -
Monitoring care quality through data review, chart audits, and
adherence to care pathways. - Coordinating services across the care
team and referring to additional resources when appropriate. -
Participating in daily huddles and serving as a medical resource
for the team. - Maintaining a professional, approachable, and
collaborative practice culture. - Completing timely and accurate
medical documentation. - Upholding organizational policies,
procedures, and quality expectations. - Collaborating with other
departments on patient outcomes, process improvements, and clinical
operations. - Identifying opportunities to expand or enhance
clinical services. - Ensuring compliance with all licensing,
credentialing, and regulatory requirements. - Building positive
relationships with community partners and healthcare collaborators.
- Working 100% onsite, with occasional movement between nearby
clinics as needed. - Performing additional duties as assigned. What
We’re Looking For: - Current and unrestricted Arizona medical
license. - Graduation from an accredited MD or DO program. -
Excellent communication and interpersonal skills. - Strong
commitment to integrated, team-based care. - Ability and
willingness to adapt to a value-based care model. - Exceptional
patient service and relationship-building skills. - Basic computer
proficiency. - Active BLS certification. - Current fingerprint
clearance card. - Active DEA license.
