Job Description:
We are seeking a dedicated RN for full-time home health care,
providing personalized assessments and developing care plans for
pediatric and adult patients.
Conduct comprehensive patient assessments and coordinate with
caregivers and physicians.
Deliver intermittent skilled nursing care in patients' homes and
supervise paraprofessional staff.
Maintain accurate documentation and ensure compliance with all
regulations.
Communicate effectively across care disciplines and adhere to
agency policies.
Qualifications include an active Colorado RN license, at least one
year of home health experience (pediatric preferred), CPR
certification, a valid driver--------s license, reliable
transportation, and successful background check.
