We are looking for an RN Hospice Case Manager to join us at OnCare AZ to build relationships and support the wellness of our residents.



Our Nurses

• 
  
• Build positive relationships throughout the home health team and community.
  
• Work closely with patients, families, and physicians to provide a personalized care experience by assessing individual needs.
  
• Train others on new principles and methodologies of care to incorporate into clinical practices.
  
• Maintain records and confidentiality regarding all aspects of patient care.
  
• Initiate and revise care plans, and supervise and monitor CNAs and CMAs to meet the needs of patients.
  
• Administer medications, maintain records and ensure care is provided to enhance the lives of our residents and meet state and federal guidelines.
