We are looking for a Physical Therapist to join our office in Peoria, AZ to provide patients with all phases of physical therapy. We are flexible in our need and can accommodate a PT who is looking for either full time or part time work.

We are looking for a dedicated and motivated Physical Therapist who is passionate about improving the health and quality of life of our patients. Our ideal candidate is compassionate, a people person who is a team player- collaborating with the team to provide the highest quality care, helping our patients reach their health and wellness goals, effectively and efficiently. We are looking for a strong multi-tasker, with great interpersonal communication skills with patients and staff members, detail oriented, and works well in a collaborative team environment.

Duties:
Patient Consultation-review health and medical histories, exam patients, review pertinent medical records-imaging, and reason for visit
Consult with referring Physicians-Healthcare team regarding diagnosis
Develop and implement physical therapy treatment programs based on each patient's particular needs- focused on pain reduction and improving functional mobility?
Assist patients with all phases of physical therapy treatment and techniques to include exercise instruction, manual therapy techniques, modern modalities, and activities of daily living
Compose discharge notes for patients, refer to appropriate outside sources, as necessary
Patient education on the benefits of PT, treatment plan maintenance, and home exercises
Documentation-progress notes- accurate and timely records, document treatments rendered in patient charts with corresponding CPT codes, Record patient conditions and responses to treatments in charts after each visit
Collaboration with the MDs, NPs, and healthcare professionals for comprehensive patient care to managing pain and improving functioning
Communicate effectively with both the staff and patients
Create an excellent experience for patients through a friendly and focused attitude
Provide direct 1-1 care to Medicare patients
Supervise PTA-PT Technicians to ensure all therapy services meet the patient's needs?
Oversight of delivery and patient education on all DME issued to Medicare patients
Assist in developing and implementing new programs-techniques to improve PT services?

Requirements:
Graduation from an accredited Physical Therapy program
PT license in AZ
Doctorate (preferred), new graduates encouraged to apply

Schedule:
Full time or part time
Clinic hours: Mon-Wed 8-12pm and 2-6pm, Tues 8-12pm, Thur 2-6pm and Fri 8-1pm
Part time M-W-F

Compensation:
$100k plus per year
